You have to pick one or the other.
And you really want to have a strong demesne because it gives you more money and troops and helps handle revolts but only having one or two sons can also be troublesome when they die unexpectantly or have bad stats/traits. Your counties will get split between your children even if you do only have one king or duchy title. They'll just stay your vassal so all the lands stay in your realm but not yours directly.

You have to keep your family small to stay powerful. It only gets worse the bigger your realm gets and the more vassals you have plotting against you and forming factions.
